✔️ Overdose Effects: Teana 20
- Overdose may lead to symptoms like hypotension, dizziness, tachycardia, bradycardia, increased serum creatinine, and acute renal failure, due to parasympathetic stimulation.
✔️ Pharmacology
- Telmisartan is a non-peptide angiotensin-II receptor antagonist. It works by blocking the binding of angiotensin-II to the AT1 receptor, which plays a key role in blood pressure regulation. This receptor is present in various tissues, such as the blood vessels and adrenal glands.
- By blocking angiotensin-II, Telmisartan prevents vasoconstriction, aldosterone release, cardiac stimulation, and sodium reabsorption in the kidneys.
- It has a significantly higher affinity for the AT1 receptor compared to the AT2 receptor and does not interfere with the angiotensin-converting enzyme (ACE), which affects the bradykinin pathway.
✔️ Common Side Effects: Teana 20
- For Hypertension: Sinus pain, sinus congestion (sinusitis), back pain, and diarrhea.
- For Cardiovascular Risk Reduction: Intermittent claudication and skin ulcers.
✔️ Dosage and Administration: Teana 20
- Starting Dose: The usual initial dose is 40 mg once daily.
- Dose Range: Effective blood pressure response is observed between 20-80 mg per day. The maximum reduction in blood pressure is typically seen after 4 weeks of treatment.
- If additional blood pressure reduction is needed, a combination therapy can be considered.
- Elderly or Renal Impairment: No initial dose adjustments are necessary, but blood pressure should be closely monitored in patients on dialysis due to potential orthostatic hypotension.
- Hepatic Impairment: Telmisartan should be used with caution in patients with liver issues, and dose adjustments may be required. Initial therapy is not recommended for patients aged ≥75 years or those with hepatic impairment.
✔️ Pediatric Use: Teana 20
- The safety and effectiveness of Telmisartan have not been established in pediatric patients.
✔️ Geriatric Use: Teana 20
- No significant differences in safety and effectiveness were found between elderly and younger patients.
✔️ Hepatic Impairment: Teana 20
- In patients with biliary obstructive disorders or hepatic insufficiency, Telmisartan should be administered carefully and with gradual dose increases.
✔️ Drug Interactions:
- NSAIDs: Concurrent use of NSAIDs may increase the risk of renal impairment and reduce the antihypertensive effect of Telmisartan.
- Aliskiren: Co-administration with Aliskiren is not recommended, especially in diabetic patients.
✔️ Contraindications:
- Hypersensitivity: Known allergy to Telmisartan or any of its components.
✔️ Pregnancy and Lactation:
- Pregnancy Category C in the first trimester and D in the second and third trimesters. It is not recommended during pregnancy due to potential harm to the fetus.
- If breastfeeding, a decision should be made whether to discontinue nursing or the drug, considering the mother's needs.
✔️ Precautions and Warnings:
- Fetal or Neonatal Exposure: Avoid use during pregnancy, especially during the second and third trimesters.
- Hypotension: Can cause low blood pressure, particularly after the first dose.
- Liver or Kidney Impairment: Use cautiously and monitor kidney and liver function.
- Avoid Combined Use with ACE Inhibitors: Using Telmisartan with an ACE inhibitor or other angiotensin receptor blockers should be avoided due to an increased risk of adverse effects.
